Hair Transplant Procedure
Hair loss is caused by a combination of ageing, hormonal changes and a genetic history of baldness. The earlier hair loss begins, the more severe the baldness. It can also be caused by trauma, or burns, whereas this surgery is considered reconstructive.
Procedure Types
Hair Transplantation - The procedure consists of resurfacing bald areas of scalp with hair. Plugs of bald scalp are removed and then filled with plugs of scalp containing several hair roots taken from hair-bearing areas. Those grafts root themselves into their new locations and hair follicles start to grow eventually.
Depending on the degree of baldness, the number of grafts needed and the length of treatment sessions vary. Several operations may be necessary. The technique may leave many small scars on the site used (donor sites) but are usually not noticeable.
Scalp Flap Transfer – This is done when the sides of the scalp (above the ears) and the back of the scalp is hair-bearing. A long thin "flap" of scalp that is hair-bearing is removed and placed across a bald section to create a band of normal hair growth. As part of the treatment, parts of the bald scalp may be removed. The donor site is closed by stretching the opposite side of the scalp.
This procedure replaces hair across a large area of bald scalp. The hair growth looks normal, and the narrow scars are hidden between the hair follicles.
Scalp Reduction (Serial Excision) – This is the removal of as much of the bald section as possible and done in the first operation. The adjacent hair bearing areas of scalp are pulled in close to the bald section, with the understanding that some bald areas will remain. This technique is repeated one or more times at a later date to eventually reconstruct the bald area.
Tissue Expansion – A device called a tissue expander is placed under a hair growth area situated adjacent to a bald area. After several weeks, the tissue expander causes skin to grow new skin cells. Then another operation is necessary to place the newly expanded skin over the ajoining bald area.
Ideal candidates for hair replacement must have a healthy growth of hair at the back and sides of the head. The hair on the back and sides of the head will be the donor sites where the flaps and grafts will be surgically removed.
The procedure location options may include the surgeon's office-based surgical facility, outpatient surgery center, hospital outpatient, or hospital in patient.
The anesthetic options are either general, or local (combined with a sedative)which allows the patient to remain awake but relaxed.
To achieve desired fullness, several surgical sessions are needed. There is a healing period of several months recommended between each session. The final result with a full transplant series may take up to 2 years.
A month or 2 after surgery, the grafted hair falls out (which is normal and temporary). It takes another month or more before hair growth starts. To create more natural-looking results, a surgical touch-up procedure may be necessary. This may consist of using a combination of mini grafts or slit grafts to fill and blend in the hairline.

Bulgaria By bus Certainly the cheapest and fastest way to travel around the country is by bus. Buses go from and to every bigger city (you might have to ask or be driven by taxi to the bus station) quite frequently (exact timetables information could be found at http://www.bus.light-bg.com/english/input_engl.html); however, most bus station agents (except at the Black Sea and in Sofia) as well as the drivers will not speak or understand any languages except Bulgarian (and, if you are lucky, Russian) and the destinations will be written exclusively in Cyrillic. Bulgaria By car If you want to reach Bulgaria from Western Europe by car, you either can take a ferry from Italy to Greece, or you will have to pass through either Serbia (make sure you took a green card from your national insurance company) or Romania.
Travelling from Greece you have to go from Thessaloniki towards Serres and then to Promahonas. Besides the sticker (see end) you need to pay the Bulgarian authorities health insurance (2 euros per person for 3 days, slightly more for more days) and car disinfection costs (4 euros for a standard car). Beware. It is common that while Bulgarians do get a receipt naive foreigners don't! Expect long queues on certain days!
The security situation regarding car theft shouldn’t be overrated. In small villages or the country, leaving your car should be safe, but in the big cities or the tourist spots it is advisable to improve your odds by parking either on the major streets or on guarded garages, where fees range from 6 lvs a day to 2 lvs an hour. If you plan to spend more time in one city, it might be better to get an abonnement, which on the average costs 60 leva a month. Most of the hotels have an own parking, and even at private lodgings it is often possible to park the car in the garden or so, just ask.

- UroToday.com - Borawski, et al in the January 2008 issue of The Journal of Urology collected 24-hour urine samples from 48 healthy subjects with no history of stone disease, endocrine abnormalities, or urological surgeries over a three and a half year period. All the children were toilet trained and age 3 to 18 years of age.
- Some nicotine replacement therapies (NRT) that were previously licensed in the UK for abrupt quitting from smoking have recently been granted a new licensed indication called 'cut down to stop' or 'cut down to quit' (CDTQ). This is aimed at smokers who are unwilling or unable to stop smoking in the short term by allowing them to gradually cut down their smoking over an extended period while supported by NRT so that they may eventually be able and willing to try to quit altogether. This systematic review examines the effectiveness and cost-effectiveness of NRT for CDTQ.
No RCTs specifically addressing CDTQ were identified. Seven randomised placebo-controlled trials satisfied the inclusion criteria; six of these were industry sponsored. The review concluded from a meta-analysis of the findings of these studies that “NRT is an effective intervention in achieving sustained smoking abstinence for smokers who declare unwillingness or inability to attempt an abrupt quit.” The authors note that the 12-month sustained abstinence success rate in this population (approximately 5.3% NRT vs approx 2.6% placebo) was considerably less than that documented for an abrupt quit NRT regime in smokers willing to attempt an abrupt quit with NRT (approx 16% NRT vs. 10% placebo). They add that as most of the evidence of effectiveness of CDTQ in this report came from trials that required considerable patient–investigator contact, a similar mode of delivery in a real-world setting would probably be needed for CDTQ with NRT to generate similar abstinence rates for this population. Furthermore, CDTQ was considered highly cost-effective compared with a no quit attempt.
